If you are on regular medication, you will have a repeat prescription slip given to you.  Requests are made by handing the slip in the reception, posting to the surgery or by putting in the post/letter box or by ordering online.  If you do not have a slip, please write down your request, including your name, address and medication required request forms available at reception.

Please do not phone us for your repeat medication.

Your doctor will review your medication at regular intervals.
